Prosperity Counseling and Supervision, LLC accepts most insurance plans. For more information about plan participation, call the office number, 734-480-8099, with any questions or concerns that you may have. If your plan is not accepted, Prosperity Counseling and Supervision, LLC is also an ‘Out of Network’ Provider. To determine if you have ‘Out of Network’ coverage, check with your insurance provider and ask the following questions:
What is my deductible and has it been met?
How many sessions per calendar year does my plan cover?
What is percentage of reimbursement?
Is approval required from my PCP (primary care physician)?
Payment of service is expected at time of session. Completed claim forms will be provided directly to you for submission to your insurance carrier for reimbursement directly to you. Contact your insurance carrier directly for information regarding rules, and regulations regarding reimbursement. For claim form to be submitted, a mental health diagnosis for psychotherapy services must be given. Not all diagnosis guarantee reimbursement. For this reason may clients elect to pay in cash to ensure their full confidentiality. For more information regarding this, please ask.
Cancellation
If unable to keep your scheduled appointment, 24-hour notice is required so the needs of all clients of Prosperity Counseling and Supervision, LLC can be met.

Please text messaging for any form of communication as they will not be responded to. They are not secure means of communication as per HIPAA privacy laws. Telephone communication is the only accepted means of communication.
